The forms required for participation in athletics are linked below. Please return all forms to Coach Moore at Weber. You can simply put them in an envelope addressed to Coach Moore and drop it at the front desk.

In order to be cleared to play, every player must turn in 1) A yearly Physical  2) Athletics Expectations form 3) Consent to Treat form 4) Participation Permission.
