Weber Sports Medicine is committed to providing complete and personalized care for all student-athletes within the athletic department. Working with the team physician and physical therapists, we will deliver optimum healthcare to ensure the safety of participating athletes through prevention, injury management, and rehabilitation.
Please download, print and have a physician sign the following forms. All forms must be completed and submitted through PlanetHS prior to athletic participation.
1.41 Students must have a certificate of an annual physical examination on file at the school prior to participating in any athletic try-outs, practices, voluntary workouts or games that indicate the students are physically approved for participation.
(a) Physical examinations will be good for twelve (12) months from the date of the exam. EXCEPTION: Any physical examination taken on or after April 1 in the preceding year will be accepted for the entire next GHSA school year.
Paige Fossey, Head Athletic Trainer, is employed at PT Solutions Physical Therapy. She grew up in Chesterton, IN, and received her Bachelor's Degree in Athletic Training from Lewis University. During her time at Lewis University, Paige also participated in their collegiate softball team. She has spent time as an athlete, a coach, and an athletic trainer. Paige is also an SSA instructor, teaching the Sports Medicine class as well as certifying students and coaches in CPR training. When she is not working with Weber athletes, Paige is enjoying the outdoors with her two dogs.
The Weber School, a Jewish Community high school serving students from all Jewish backgrounds, prepares students for success in college and in life with comprehensive academic and co-curricular programs that inspire student exploration, leadership, and Jewish social consciousness.
